Home Care Service
The Home Care Services can be of benefit to the person with dementia at any stage of the disease. It is a flexible, personalised service which reflects a person centred approach to care as all activities are based on the client's and carer's needs, interests and wishes.
The focus of the service is to support the person with dementia in engaging in activities at home and enabling the person to remain in their community as long as is possible while simultaneously protecting his or her rights. The service also offers the main carer or family member a break and access to information and support.
The home care service assigns a trained dementia care worker to provide support and care in a person's residence for a designated number of hours per week. The care worker will support the person with dementia in engaging in activities and personal care which they may be no longer able to do on their own.
How to Access the Service:
Call the Home Care Coordinator operating the Home Care service nearest to you.
Before a care worker goes into the home, the needs of the person with dementia and their family are carefully assessed by the Home Care Coordinator who meets with the family to determine how best the service can help.
This assessment involves the completion of a referral form where a doctor or consultant confirms a diagnosis of dementia, such as Alzheimer's disease, has been made and the development of a care plan for the person which is based on their preferences and needs.
