Case Management Service
The Society operates a Case Management Service in the HSE Dublin South West area.
The service aims to support carers and people living with dementia who would benefit from having someone work with them to access services and supports in their area.
This service is open to people with dementia, their families and carers who live in the following areas: Greenhills, Crumlin, Drimnagh, Fortunestown, Kilnamanagh, Tallaght and Walkinstown.
Referals to the service can be made by health care professionals such as doctors, public health nurses and social workers, and directly by people with dementia and their familes. A referral form is available to download below.
